U-12 Girls Tryouts

Tryouts for the Girls U-12 team is scheduled for 10am to 12 noon on May 6th at Crystal Lake Sports Park.
Please bring the following:
- Completed Tryout registration form for each child
- $50 tryout fee
- 100% of which goes to the CFA Scholarship fund.
- The fee can be waived be filling out the Scholarship form.
- Youth’s birth certificate
- Youth’s proof of health/accident insurance
- We will also have a Notary at tryouts so you can fill out a new Medical Release form at the field.
Practices for U-12 Girls will start Wednesday May 9th 4:00-5:30pm at Crystal Lake Sports Park.
