To apply for a position in the Corvallis Football Academy, please provide the following completed documents at your scheduled tryout:
- Youth’s Tryout Application (50k .pdf)
- Download, print, and fill in with pen.
- Youth’s birth certificate
- Youth’s proof of health/accident insurance
- Medical Release Form
- Download, print, and fill in with pen.
Scholarships are available for those in need. Please also bring the following form to the Tryout if you need assistance with fees:
